Please Note:  A JV/Varsity student athlete is NOT cleared to play until approved by the school nurse. A valid physical must be on file before being approved to play.

Medical alerts such as asthma or food allergies require a current emergency care plan (with both doctor and parent signature). Please check with the nursing office if you are unsure as this may delay your child's clearance.
