Important Forms for Clubs
Fundraising Request Form
Fundraising request forms should be filled out two weeks prior to the fundraising activity.
Profit and Loss Statement
Profit and Loss Statement for all sales. This form is to be completed at the end of every fundraiser and submitted to the Central Treasurer. Please print in portrait mode.
Admissions reconciliation form to be completed at the end of every ticketed event and submitted to the Central Treasurer upon completion.
